A Functionalist parish church of 1936-1937 in Kozłowa Góra, designed by Jan Affa.
The Church of Our Lady of Perpetual Help is a Roman Catholic parish church in the Piekary Śląskie deanery of the Archdiocese of Katowice. It stands in Kozłowa Góra, a district of Piekary Śląskie.
The church was built in 1936-1937 on the site of a small church of 1907, to a design by the Katowice architect Jan Affa, in the Functionalist style. In the main altar is a painting of Our Lady of Perpetual Help, which also shows two churches: the newly built one and the church of St Adalbert in Radzionków. The side altars are dedicated to St Barbara and the Sacred Heart of Jesus. Until 1937 the church in Kozłowa Góra was the church of a local chaplaincy, and since 28 May 1957 it has been a full parish church.
